How to get form 063/o?
First, you need to schedule a consultation with a pediatrician or family doctor. During the visit, the specialist will review information about the preventive vaccinations received, examine the medical documentation, and, if necessary, provide recommendations regarding further vaccination according to the National Immunization Schedule of Ukraine.
If all required vaccination records are available, the doctor will issue Form 063/o, which contains information about the child's preventive vaccinations. If certain vaccinations are missing, the doctor will help create an individual vaccination plan and select an optimal immunization schedule according to the child's age and health condition.
To schedule a consultation, call +380 97 312 92 91 or visit Doctor Sam Medical Center at 1 Kiltseva Road, Kyiv (Respublika Park Shopping Mall).
FAQ
What should you do if the vaccination record is lost?
Losing a vaccination record is a fairly common situation, especially when moving, changing healthcare providers, or transferring a child to a different school or kindergarten. However, in most cases, information about previous vaccinations can be restored.
To do this, the doctor reviews available medical records, discharge summaries, previous certificates, and other documents that may contain information about vaccinations received. If necessary, data from the electronic healthcare system or information from a previous healthcare provider may also be used.
If some vaccination records cannot be confirmed with documentation, the doctor will provide recommendations on the next steps and help determine a safe vaccination strategy according to the child's age and current medical guidelines.
