Declaration with a family doctor


A declaration is your choice of a primary care doctor – the doctor you can turn to with most health-related concerns. For adults, this may be a family doctor or a general practitioner, while for children, it may be a paediatrician. At Doctor Sam, family doctors provide care for both children and adults. Once you sign a declaration, a defined range of primary healthcare services is funded by the National Health Service of Ukraine (NHSU). This means that consultations, diagnostic procedures and other services included in the primary healthcare package and provided based on medical need are free of charge for the patient. The same principle applies to private medical centres that have a relevant contract with the NHSU.


What does a declaration provide?


Depending on your health condition and medical needs, a family doctor can:

  • provide consultations and treat the most common conditions;
  • monitor your health over time, including the management of chronic conditions;
  • carry out preventive check-ups;
  • monitor children’s health and development;
  • provide vaccinations in accordance with the National Immunisation Schedule;
  • prescribe and monitor necessary tests and examinations;
  • issue electronic prescriptions, including prescriptions under the “Affordable Medicines” programme;
  • create electronic referrals for specialist consultations, laboratory tests and other investigations when needed;
  • issue certain medical documents within the doctor’s scope of practice.


The guaranteed package also includes a defined range of laboratory and diagnostic tests, such as complete blood and urine tests, blood glucose and cholesterol testing, ECG and other investigations, depending on the medical centre’s available services and medical indications.


It is important to understand that having a declaration does not mean that every appointment or test is free of charge. A declaration gives you access to services included in the primary healthcare package and the Medical Guarantees Programme. The doctor determines which consultations, tests or referrals are necessary based on your individual situation and medical needs.


What is the advantage of declarations?


Without a declaration, each new health concern usually starts with finding the right specialist and booking a separate appointment. A declaration allows you to choose a doctor you or your child can turn to with a wide range of primary healthcare needs.


This is particularly convenient when an initial medical examination and an understanding of the next steps are required. A family doctor evaluates your symptoms and determines whether the issue can be managed at the primary care level or whether additional tests or a consultation with a specialist are needed. For this reason, a declaration can be useful for regular preventive care and the management of chronic conditions.


If you or your child need a one-time appointment without signing a declaration, you can book a family doctor consultation for children or a family doctor consultation for adults.


Can you sign a new declaration if you already have another doctor?


Yes. Patients are free to choose their primary care doctor. If you decide to change your doctor, you simply sign a new declaration, and the previous one is terminated according to the established procedure. A declaration is not tied to your place of registration or residence, so you can choose a doctor where it is most convenient for you to receive medical care.

Online consultation or a doctor’s home visit: which option is right for you?


If your condition makes it difficult to come to the medical centre, a consultation at home may be a suitable option. An online consultation can be convenient if you need to discuss test or examination results, clarify the next steps, talk about changes in your condition or receive medical advice in a situation where a physical examination is not necessary. If, during the consultation, the doctor determines that your condition cannot be properly assessed without an examination, they will recommend an in-person appointment.


A family doctor’s home visit may be appropriate when it is difficult for the patient to come to the medical centre, but an in-person assessment is still necessary.
